MALARIA: Spain: Historical aspects


Every summer thousands of us travel to exotic climes with mosquito nets, jungle-strength cream and anti-malarial drugs. But how 'foreign' really is Malaria to Spain and Europe ?
Despite all the concern about climate change enabling malaria to spread to the West, the weather is by no means the biggest factor in determining the presence of malarial mosquitoes. Europe was until the last century rife with the disease. In fact, the last country in Western Europe to eradicate malaria was cold, damp Holland in the late nineteen-sixties.
Malaria, or 'paludismo' as it is also known in Castilian, was endemic to Spain until well into the 20th century. Its existence has had a huge effect on the landscape in certain areas. Much of Spain 's wetland surface area has been drained in the fight to eradicate it. On the flip side, some sites like Doñana have to some extent been saved from extensive rice farming by the presence of malarial mosquitoes.
The disease probably took off from its ancestral enclaves with the Neolithic revolution between 8,000 and 10,000 BC. Sedentary life, the formation of the first villages, land clearance and crops irrigation, the increase in human population (thanks notably to the improvement in living conditions and food availability) certainly favoured the spread of malaria. New strands of resistant parasites would have been brought by the waves of invasions that swept across the Peninsula . We know for example that malaria followed Hannibal in his wake.
By the Middle Ages, kings and noblemen had gained control of the best wetlands for themselves, where they could hunt and earn lucrative profits by exploiting their natural resources (everything from rice cultivation to leech farming). However, these advantages were offset by the fear of marshes as breeding grounds of plagues and incurable fevers. As way of illustration, while the common European word comes from the Latin for 'bad airs', 'paludismo' comes from the Latin 'Palus' for lagoon. Such was the dread of these sites that a royal decree was passed in 11th-century Valencia sentencing any farmer to death who planted rice too close to villages and towns. For centuries afterwards, there was conflict between rice growers and the authorities who passed law after law restricting rice fields to wetlands that were unsuitable for other types of crops.
The disease continued to decimate local populations throughout the centuries and was to spread over the centuries with the increase in rice farming. The only areas relatively free of the disease were the colder northern statelets (Asturias, northern Navarra and Leon.). This north-south divide was patent in the organisation of the ventures of the Conquistadors; soldiers from Asturias , Galicia or Vizcaya would often be rejected for their tendency to suffer chapoteadas - marsh fevers. It was believed that people from malarial zones had developed a certain lucky immunity (I am unaware as to whether there is any real immunity among Spaniards as this is say among Sub-Saharan Africans). There is a long list of famous people who have died here from malaria ( Hannibal 's wife and son-see above, Emperor Isabel, Felipe II, Felipe IV, Felipe V, Fernando VI, Carlos II, Santa Teresa de Jesús, Hernán Cortés.).

An outbreak hit Barcelona in the 1880's as the city ran out of money to finish the Eixample, as is described by Robert Hughes in 'Barcelona'. As speculation sent prices sky high, thousands of investors went bust when the bubble burst and hundreds of plots were left bare for a decade. Here stagnant waters built up, a ripe environ for mosquito larva.
At the turn of the 20th century, malaria was considered the biggest single health risk by the Spanish authorities, and an estimated 800,000 people had malaria in Spain , with some 4,000 dying every year. This concern led to the passing of the Cambó Law in 1918, which gave legal backing to the already strong trend of wetland drainage since the mid-19th century. The law was often ineffective as it allowed for wetlands to be converted into rice fields, though it was responsible for the destruction of much of Spain 's wetland surface area until its repeal in the early 1980's.
Along with drainage, one of the most effective controls was the release in 1921 of a fish called 'gambusia' or mosquitofish, incidentally probably now the most widespread freshwater fish in the world. This little fish is a voracious devourer of mosquito larva and rapidly took to Iberian waters. Improvements in housing, public health and sanitation, and a falling rural population all helped to cut back the parasite, though the Civil War meant a temporary halt to its retreat. Four years after it finished, in 1943, a final serious outbreak hit the country with 400,000 people affected and 1,250 deaths, but by the end of the forties it had been effectively controlled and restricted to a few pockets, with the use of DDT from 1947 onwards delivering the coup de grace .
The defeat of malaria
Malaria was declared officially eradicated in 1964, which was just in time for mass tourism, which certainly would not have taken off had it not been for the parasite's prior eradication. It also coincided nicely with the UN no longer classifying Spain as a Third World country. As all development workers know, malaria is eradicated by means of progress, not by a change in the temperature or necessarily draining all wetlands.
Malaria could indeed return to Europe , but the real trigger would be a massive economic meltdown rather than any rise in temperature, which would only make matters worse.

Malaria update
Nothing to do with Spain, but following on from the 'Malaria in Spain' thing, I've discovered that Holland wasn't the last place in Europe to be declared malaria-free. Thus: It was not until 1975 that the last pocket of indigenous malaria in Greek Macedonia was considered eliminated and the World Health Organization declared the continent free of the disease. However, so called 'imported' as opposed to autochthonous malaria is an increasing problem in Spain and the EU. According to the WHO
Throughout Europe, imported malaria is a growing medical and health issue. Since the early 1970s, the reported number of imported cases increased ten-fold, from 1 500 cases in 1972 to more than 15 000 in 2000.

POVERTY: Quarter of Spanish children at risk of poverty

One in four children in Spain live in homes at risk of poverty, Unicef said Tuesday in a report that identifies the offspring of immigrants as 'especially vulnerable'.
While in 2006 the poverty rate for households with children born in Spain was 21.2 percent, it rose to 52 percent in the case of children born outside the European Union, a difference that becomes even more acute in cases of severe poverty, where the comparable figures are 4 percent and 28 percent, respectively.
The report thus places Spain among the countries with the highest juvenile poverty rate among the 27 countries of the EU, with only Romania, Bulgaria, Latvia and Italy in worse condition.
'Being a poor child in Spain doesn't mean going hungry, but malnutrition is a definite possibility; it doesn't mean having no access to education, but it does mean having trouble paying the expenses and dropping out of school at an earlier age; it doesn't mean not being able to see a doctor, but paying for some treatments is a problem,' Unicef's Marta Arias said here at the presentation of the report.
The study 'Childhood in Spain 2010-2011' reveals that 24.1 percent of minors live in homes with an income below 60 percent of the national average, which for a family of two adults and two children stands at 16,000 euros ($22,000) a year.
In single-parent households and in families with two or more children, the poverty rate soars to between 19 and 25 percentage points, according to the report, which points to other factors besides being immigrants that make children particularly vulnerable: belonging to a minority, being handicapped, or living with disabled people.

MALARIA: 1st indigenous case of malaria in Spain in nearly 50 years

The last time the country of Spain had seen an indigenous case of malaria was 1961, until last month. A patient with no travel history from the Huesca province has been confirmed with an infection with the parasite, Plasmodium vivax.
Spanish health authorities call this an “epidemiological curiosity” and say the case poses no risk to the public.
Health officials say that the individual contracted the through the bite of an Anopheles
atroparvus mosquito.
Reports say that the patient has been treated and is recovering.
Malaria is not rare in Spain with upwards of 500 cases seen annually, however all cases are attributed to traveling abroad and are considered “imported”.
The specific species of malaria in this case, Plasmodium vivax, typically is a relatively mild form of the disease and is not usually fatal.
Malaria was officially declared eradicated in Spain in 1964.

MALARIA: Spain: Malaria resurgence risk in southern Europe

Background
International travel and immigration have been related with an increase of imported malaria cases. This fact and climate change, prolonging the period favouring vector development, require an analysis of the malaria transmission resurgence risk in areas of southern Europe. Such a study is made for the first time in Spain. The Ebro Delta historically endemic area was selected due to its rice field landscape, the presence of only one vector, Anopheles atroparvus, with densities similar to those it presented when malaria was present, in a situation which pronouncedly differs from already assessed potential resurgence areas in other Mediterranean countries, such as France and Italy, where many different Anopheles species coexist and a different vector species dominates.
Methods
The transmission risk was assessed analysing: 1) climate diagrams including the minimum temperature for Plasmodium falciparum and Plasmodium vivax development; 2) monthly evolution of the Gradient Model Risk (GMR) index, specifying transmission risk period and number of potential Plasmodium generations; 3) ecological characteristics using remote sensing images with the Eurasia Land Cover characteristics database and the monthly evolution of the Normalized Difference Vegetation Index (NDVI); 4) evaluation of A. atroparvus population dynamics.
Results
Climatological analyses and GMR index show that a transmission risk presently exists, lasting from May until September for P. falciparum, and from May until October for P. vivax. The GMR index shows that the temperature increase does not actually mean a transmission risk increase if accompanied by a precipitation decrease reducing the number of parasite generations and transmission period. Nevertheless, this limitation is offset by the artificial flooding of the rice fields. Maximum NDVI values and A. atroparvus maximum abundance correspond to months with maximum growth of the rice fields.
Conclusions
The Ebro Delta presents the ecological characteristics that favour transmission. The temperature increase has favoured a widening of the monthly potential transmission window with respect to when malaria was endemic. The combined application of modified climate diagrams and GMR index, together with spatial characterization conforms a useful tool for assessing potential areas at risk of malaria resurgence. NDVI is a good marker when dealing with a rice field area.

TUBERCULOSIS: Vaccine development in Germany

Professor Stefan Kaufmann of the Max-Planck-Institute for Infection Biology in Berlin, Germany, illustrated that TB infection is a vicious circle: Each day about 125,000 infections result in roughly 25,000 TB cases, or 10 million new cases of TB disease a year. Around 5,000 people die of tuberculosis every day. Drug resistant TB strains, and HIV/TB co-infection further challenge global TB control.
Kaufmann emphasized that vaccines can play an important role in turning the tide. He referred to studies showing that a 40-50% reduction in TB could be achieved by new vaccines. Improved drugs could lead to a 10-27% reduction and better diagnostics to a 13-42% drop.
He elaborated on the development of the VPM1002 vaccine candidate by his team. VPM has successfully completed tests on safety and immunogenicity in Germany. Further trials (phase Ib) are taking place in South Africa at the moment.
Also in Spain progress is made. “It is a difficult process from research to development,” Professor Carlos Martin of the University of Zaragoza said, describing the development of the MTBVAC01 vaccine candidate discovered by him and his colleagues. After more than ten years of discovery and proof of concept and now four years of development, the vaccine is taking its first steps out of the lab: The vaccine is scheduled to be tested in people (phase I safety trials) the end of next year.
Dr Jelle Thole, director of Tuberculosis Vaccine Initiative (TBVI), in a meeting with Spanish journalists called MTBVAC01 a leading vaccine in its kind. “It is the only candidate derived from the actual bug that starts tuberculosis. All other vaccine candidates either are based on improving or boosting BCG,” he explained. TBVI, a European research consortium for the development of new TB vaccines, aims to have eight vaccines in phase II safety and efficacy trials in ten years.
New vaccines are aimed to improve or replace BCG, the only currently available vaccine against tuberculosis, which “does not protect against the most prevalent form of the disease, and therefore has little - if any - impact on the epidemiology of TB,” Kaufmann said.
However, Martin stressed that BCG, developed in the 1920s, is “…still in use because it is protective in children. In this regard, new vaccines should be at least as good as BCG in protecting against severe diseases as meningitis and miliary tuberculosis, and better in protecting against respiratory forms of the disease.”
Worldwide, new tools against tuberculosis are in several stages of development. Jan Gheuens, a senior programme officer on the tuberculosis team at the Bill & Melinda Gates Foundation, summarized at the symposium: “Two vaccines are in large clinical trials, lots of vaccines are in other phases. Furthermore, eight drugs are in pre-clinical development or further. And there is much excitement on the performance of a TB molecular diagnostic test”.
Gheuens briefly mentioned strategic challenges to be considered. “What will be the next generation of new vaccines? New antigens or a new approach to vaccines? What about the cost of progress, can we raise the funds for larger clinical studies?” he questioned, adding that fundraising is ‘tough’ and that not just greater awareness of the challenges in TB, but also (public/political) commitment is needed”.

POVERTY: Spain's figures

More than a million more Spaniards are living below the poverty line now than before the country was battered by an economic crisis, according to a study by a Spanish charity released Friday.
Some 22.7 percent of Spain's population of around 46 million were either moderately or severely poor in the second half of 2009, when the study was carried out, up 3.4 percent from an earlier survey in 2007, the Spanish branch of the international charity Caritas said.
"It seems clear that the social impact of the crisis is far from ending," the study said. "We are talking about the first impact of the crisis over the two years, which will have to be monitored in the future."
The Spanish economy entered recession at the end of 2008 as the global financial crisis accelerated a collapse of its once-booming property sector. It only emerged with tepid 0.1 percent growth in the first quarter this year.
The crisis has sent unemployment soaring to more than 20 percent, the second highest in the European Union after Latvia.

Global Agriculture and Food Security Program

A global economy where more than one billion people suffer from hunger is not a sustainable one," US Treasury Secretary Tim Geithner said in a ceremony launching the Global Agriculture and Food Security Program.Canada, Spain, South Korea and the United States, along with the Bill and Melinda Gates Foundation, have already pledged nearly 900 million dollars to the program, initiated as the global economic crisis leaves even more people affected by poverty and hunger."At a time of limited resources and large global challenges, this fund will leverage support from around the world to achieve lasting progress against hunger and bolster agricultural productivity and growth," Geithner said.The fund was first discussed at the G8 meeting in L'Aquila, Italy last year, where 14 wealthy nations committed to contributing some 22 billion dollars to invest in agriculture in low-income countries.It firmed up at the subsequent G20 meeting in Pittsburgh, where world leaders called for the World Bank and interested donor nations to set up a trust fund to help implement some of the pledges made at L'Aquila.The United States said Thursday that 67 million dollars of its initial contribution to the fund will be transferred in the coming weeks to the World Bank, which will administer the fund.
